Position: Social Support Coordinator

This full-time position provides structured group activities to help isolated, lonely older individuals the opportunity to socialize with their peers by leading one on one interactions, and small or large assisted socialization groups. The Coordinator establishes relationships with participants by visiting in the home, inviting new referrals to try the group, breaking down barriers to joining in, facilitates guided group discussions to encourage participation, and provides resources for participants when unmet needs are identified. The coordinator will assist with planning a calendar of outings based on participant needs and requests, transport people on outings, and document case notes and billable hours in real time.

The preferred candidate has health, social work, or related education or experience. The Coordinator’s schedule includes time in the community, in client homes and in the office.
