Position: Outreach Coordinator

This full-time position assists in improving client quality of life and breaking down barriers to service. As members of our community, the coordinator becomes familiar and partners with local resources, which helps to build trust and develop relationships with clientele. In the Caring Services department, the Outreach Coordinator works closely with the Director of Client Services and the point-of-referral to complete the following responsibilities: screening new referrals, completing comprehensive client assessments and re-assessments as needed, assisting with barriers to service, and crossing referrals to USS services and other appropriate service providers. The Coordinator provides clients health education, coordinates local resources, and assists with access to needed support to maintain or improve their wellbeing. The Coordinator advocates for clients right to live in a safe environment, free from abuse, neglect, or financial exploitation. The Coordinator enters all client encounters and data into the client electronic record. Their schedule includes time in the office, in client’s homes, and other community resources.

The preferred candidate has health, social work, or related education or experience. Community Health Worker certification is a plus.
